Fiber optic and other types of private utilities are placing above ground pedestals in front of homes. Anyone have an ordinance requiring below grade vaults?

Not in my city or my experience. I know below grade exists. I can only imagine that water intrusion can be a likely issue.
 
All of ours are pole mounted or in pedestals. Even in nicer neighborhoods they just wind up putting some bushes or screening them. Oh, I forgot, we also occasionally get the giant fake rock that everyone knows is a fake rock hiding a pedestal.

Better than a fake tree hiding a communications tower.
 
Those always look like massive toilet brushes to me. We get the evergreen kind, I do like the 150 tall palm trees.
